Being entrusted with the care of a patient is a tremendous responsibility, one which we are committed to honoring. The clinicians we employ understand that home care and true case management requires much more than just taking vital signs, administering medication, changing a dressing, or assisting in physical therapy exercises. They are there to be educators, advocates, and coordinators, working to help improve the patient’s overall quality of life.
Home care is a collaborative effort. The clinicians will work in coordination with the patient, physician, pharmacy, family, and caregivers to create an individualized care plan to specifically fit each patient’s needs. Communication is vital in home care and is necessary to ensure a successful outcome.
We will be there not only to care for the patient, but just as importantly, to educate them. Taking the time to teach them how to manage their symptoms, administer their therapies, watch for warning signs, care for their catheters, reduce the risk of infection, and other skills they can use to guard their health and improve their well-being.
Our goal is to create a partnership between the clinician, patient, and caregiver so that we can maximize the advantages of receiving treatment in the comfort of your own home.
